Description
The Massachusetts Hockey Sportsmanship Award is available to students who have participated in a Massachusetts hockey program for at least five years. You must plan to receive or have a high school diploma or equivalent and plan to enroll in a post-secondary institution. Also, you must submit the following documents to be eligible for this award: two letters of recommendation, a personal statement, verification of your participation in a hockey program and a guidance counselor form.
